Pain & Injury Management

Bridging Rehabilitation and Athletic Performance

Athletic Health & Performance helps athletes transition safely and confidently from rehabilitation back to full sport participation. Our Return-to-Play program bridges clinical care and high-performance training with objective gates, sport-specific progressions, and coordinated medical oversight so athletes return stronger, more resilient, and ready to perform

Program Overview

  • Purpose: Move athletes from rehab-ready to competition-ready using measurable criteria and phased exposure to sport demands.
  • Approach: Individualized, evidence-informed, and team-driven — combining movement screening, objective performance testing, strength and conditioning, sport simulation, and ongoing medical collaboration.
  • Outcomes: Reduced re-injury risk, restored sport-specific capacities, improved work capacity, and renewed confidence under game-like conditions

Return-to-Play Services

Gradual Sports-Specific Progression

Strength and Conditioning Integration

Strength and Conditioning Integration

We design and implement a stepwise exercise progression that starts with foundational movement and advances into sport-specific skills, volume, and intensity as the athlete demonstrates readiness

Strength and Conditioning Integration

Strength and Conditioning Integration

Strength and Conditioning Integration

We apply S&C principles to improve work capacity, force production, and tissue tolerance so athletes build resilience and reduce re-injury risk

Controlled Sport Simulation

Strength and Conditioning Integration

Performance Testing and Benchmarks

We recreate sport-like movements in a controlled environment to restore technical timing, decision-making, and confidence while limiting exposure to high-risk loading too early

Performance Testing and Benchmarks

Performance Testing and Benchmarks

Performance Testing and Benchmarks

We use validated performance tests to evaluate agility, strength, power, movement quality, and other functional metrics that guide progressions and return-to-play decisions

Ongoing Monitoring for Red Flags

Performance Testing and Benchmarks

Ongoing Monitoring for Red Flags

We continuously screen for pain, swelling, instability, and other warning signs, adjusting the plan immediately if a setback or compensatory pattern emerges

Multidisciplinary Collaboration

Performance Testing and Benchmarks

Ongoing Monitoring for Red Flags

We work closely with physicians, physical therapists, athletic trainers, and other medical professionals to ensure exercise programming aligns with clinical goals and healing timelines

How it works

Initial Assessment and Baseline

Initial Assessment and Baseline

Initial Assessment and Baseline

  • Comprehensive intake and medical review including injury history, surgical notes, imaging, physician restrictions, and rehab progress.
  • Movement and load tolerance screening to safely determine which tests and intensities are appropriate.
  • Performance battery capturing sprint mechanics, strength and power profiles, agility and COD performance, aerobic or metabolic capacity where relevant, and psychological readiness.
  • Objective baseline report with prioritized deficits, symmetry thresholds, and initial phase goals

Phase-Based Progressions

Initial Assessment and Baseline

Initial Assessment and Baseline

  • Phase 1 Protected Reintegration: Re-establish pain-free movement, neuromuscular control, and low-load strength.
  • Phase 2 Capacity and Resilience: Increase strength, volume tolerance, and unilateral control with progressive metabolic and eccentric loading.
  • Phase 3 Power and Reactivity: Restore rate of force development, plyometric responsiveness, and sport-specific reactivity.
  • Phase 4 Controlled Sport Reintegration: Gradually reintroduce sport drills, unpredictability, and contact with monitored volume and intensity until full practice readiness

Objective Gates and Testing

Initial Assessment and Baseline

Communication and Documentation

  • Gatekeeping criteria tied to test outcomes such as limb symmetry index, hop tests, force output, sprint thresholds, and pain-free range.
  • Repeatable benchmarks used to authorize progression, maintain load prescriptions, or trigger regression and medical review.
  • Quantified monitoring through session RPE, wearable data when available, and regular retests at key phase transitions

Communication and Documentation

Communication and Documentation

Communication and Documentation

  • Shared RTP roadmap provided to athlete, coach, and medical team detailing phase goals, pass/fail criteria, and timeline expectations.
  • Session notes and test reports stored and shared for transparent decision-making and efficient clinician-to-clinician handover.
  • Education and informed consent for athletes and stakeholders on risk trade-offs, expected timelines, and regression rules.

Reintegration and Maintenance

Communication and Documentation

Reintegration and Maintenance

  • Stepwise practice entry moving from controlled drills to full practice and competition only after meeting clearance criteria.
  • Post-return monitoring with scheduled check-ins and retesting at 4–12 week intervals to detect residual deficits.
  • Long-term maintenance programming to convert rehab gains into durable performance and injury prevention.

Frequently Asked Questions

Please reach us at daryll@athletichealthandperformance.com if you cannot find an answer to your question.

Duration depends on injury severity, healing status, and sport demands; typical programs run 6–8 weeks from baseline to clearance when medical criteria are met

A licensed clinician or performance lead coordinates the plan and communicates with the athlete’s medical team, with support from S&C coaches and athletic trainers

Medical testing and clinician services may be eligible when prescribed by a physician; we provide documentation to support insurance where applicable

Progressions are reversible; failing a gate prompts regression to the prior safe phase, targeted remediation, and physician consultation if indicated
